Before your surgery
Surgeon consultation
You will have seen your surgeon who will have discussed the details of your eye surgery with you. This will include what risks are involved and what outcome you can expect.
Consent for surgery
Your consent form is completed either at your initial consultation or on the day of surgery. This confirms you have received all the information you require to make an informed choice and that you agree to your treatment. This also includes consent for blood sampling in the event of a staff member receiving a needle stick injury or other blood accident during your surgery/procedure.
Booking your eye surgery
Our booking clerks will arrange a date with you for your pending eye surgery, provide a copy of information about the surgery and an estimation of costs. You will also be asked to complete a pre- operative health questionnaire. If sent to you, please complete and return within 7 working days of your surgery. By receiving this information in advance we can be better prepared for your visit and ensure that you receive the best possible care.
Approximately a week prior to your surgery, a letter will be sent to you confirming your surgery time, date and the approximate length of your stay. Any special instructions for your admission and surgery will be included in the letter.
Medications
Please list all your medications including tablets, inhalers, injections, herbal remedies, vitamins or any other supplements on the pre-op health questionnaire form. Your surgery letter will include medication instructions and any special requirements prior to surgery.
Lab tests
You will be informed if you require any blood tests or an ECG, prior to your surgery.
